I will remain responsive after a referral and promptly report complaints, cancellations, disputes, or suspected misrepresentation. I understand that access may be suspended or ended for misrepresentation, unauthorized commitments, customer-care failures, security concerns, or policy violations. I consent to receive and sign this agreement electronically and understand that the company will retain an electronic record of my acknowledgement.
